Birth Certificate (Duplicate) as per Record (Born in Kuwait)

• Copy of Birth Certificate with Birth Record number (BR number) and date.
• Duly filled Form No. 2
• One recent photograph.
• Current Passport and Civil ID copy.

Please Note:

• BR number is mandatory for verification of old record.
• If the applicant is not residing in Kuwait, need to authorize relative/ friend in Kuwait to approach Embassy for the service.
• In such case, need to submit Passport and Civil ID copy of the authorized person.
• Form No. 2 may download and filled by the applicant and may send to the authorized person along with the signature as per Passport.
